Intake Procedure

Upon enrollment into a CCAF program, families are required to complete an intake interview.  The intake interview includes the following components:
  • Eligibility and Placement Review – CCAF staff will inquire about specific needs of the student to determine level of care, intervention needs, including therapy services, classroom placement, start date and specific program services to be delivered.
  • Financial Review – CCAF staff will review tuition expenses and payment options.  Parents will be given a description of financial assistance opportunities and required paperwork, if eligible to apply.  Families will be required to pay the registration fee at this time.  The registration fee is $75.00 for one child and $125 for two or more children.  The registration is required by all families, is nonrefundable and scholarship funds may not be applied to this fee.  The registration fee is used to help pay for nursing services and supplies as well as classroom supplies. 
  • Program Delivery and Policy Review – CCAF staff will review the handbook, center policies and procedures, including but not limited to diapering, nutrition and discipline.
  • Health Services Review – CCAF nursing staff will complete and interview with families to receive medical history, current medical needs and concerns, medication dispensation authorization and to coordinate nursing services with the child’s program.

  • Emergency Authorization and Contacts
  • Authorization for Pictures and Video
  • Authorization for Access to Records
  • HIPAA Acknowledgment
  • Permission for Developmental Screening
  • Parental Agreement
  • Health Intake Form
  • Family Income Verification Form
  • Getting To Know You

Immunization records and a copy of the student’s insurance card are due upon intake.
